GCOE Performance Alerts and Performance Improvement Plans
Overview
The Goodwin College of Education is implementing a new technological process for Performance Alerts and Performance Improvement Plans. The purpose of these processes remains the same: to provide timely support and clear documentation for teacher candidates.
The is designed to support teacher candidates who are experiencing challenges in meeting professional standards or demonstrating appropriate professional dispositions. It serves as both a documentation tool and an intervention mechanism to ensure that candidates receive the support needed to succeed in their teacher preparation program.
Please use this form:
- When a candidate demonstrates performance issues related to the InTASC standards.
- When professional dispositions are not being met at expected levels.
- When academic misconduct occurs.
- When early intervention may prevent more serious performance concerns.
This form should be completed as soon as performance concerns become apparent. Specific, objective evidence supporting your observations is required. Please document any interventions you have already attempted and complete all relevant sections thoroughly.
All alerts begin at the Green, or basic, level. Program coordinators and department chairpersons will review these alerts to determine whether the next level of support is necessary. Selecting the appropriate Program and Level in the form will automatically notify your program coordinator and department chairperson.

Upon receiving notification of a performance alert form from an instructional or supervisory colleague, you will determine whether the alert requires a higher level of support, based on guidance previously provided by the Dean of the Goodwin College of Education.
According to the Teacher Candidate Handbook:
- A is implemented when concerns are more significant or when Green level interventions have not resulted in sufficient improvement. This level involves teacher candidates, instructors, program coordinators, and/or department chairs in developing a comprehensive plan to address the identified concerns.
- A is reserved for situations in which there are serious questions about a candidate’s readiness to continue in the program or where concerns pose potential risks to students in clinical settings.
Yellow and Red level support forms serve as documentation of the processes undertaken by program personnel and committees.
After the required steps are taken, will be completed and sent to the teacher candidate and required administrators for digital signature acknowledgement.
In the least likely scenario, when an egregious action or event has occurred, a must be completed.
